The license for a professional counselor (LPC) requires specific training and they are required to follow the Laws & Statutes for Professional Counselors. The licensing, complaint process, and disciplinary action is governed by Texas State Board of Examiners of Professional Counselors.

The DSHS Professional Licensing and Certification Unit consists of twenty-three separate regulatory programs. The programs evaluate credentials, give examinations, monitor continuing education activities, conduct complaint investigations, and otherwise regulate a variety of health professions and related facilities in the State of Texas. Contains links to copies of complaint and general release forms, and the instructions necessary to complete the forms correctly.

I would like to formally request any information you can provide me under the freedom of information act or open records laws regarding the licensing of a social worker by the name of (Social Worker Name). She has been employed previously or is currently employed at (Name of Hospital, CPS Agency..) in (City and State).

I have not been able to find her on any online searches when looking for credentials on social workers.
